Rehab Billing

Rehab Medical Billing Services by Prospect Healthcare Solutions

Physical therapy clinic team

Rehab medical billing services cover the coding, claims, denials and payments for rehabilitation care: physical therapy, occupational therapy, speech-language pathology and multidisciplinary rehab programs. Prospect Healthcare Solutions handles this full revenue cycle for outpatient rehab clinics, rehab agencies, hospital outpatient departments and therapy groups, so your clinicians can focus on helping patients recover.

Rehab billing is one of the most rule-heavy areas of healthcare. Timed units, discipline modifiers, shared therapy thresholds, plans of care and payer visit limits all apply, often to the same patient across three disciplines. We code every visit from the documentation, apply each payer’s rules and follow every claim until it is paid.

Why It's Different

Why Rehab Billing Is More Complex Than Other Specialties

A rehab patient may see a physical therapist, an occupational therapist and a speech-language pathologist in the same week, each with their own codes, modifiers and documentation. Payers track and limit these services differently.

Common challenges include:

  • Timed codes and Medicare’s 8-minute rule
  • GP, GO and GN modifiers by discipline
  • KX threshold shared by PT and SLP, separate for OT
  • Multiple procedure payment reductions
  • Plans of care, certifications and progress reports
  • Visit limits and authorizations that change by payer

Payer Rules

Rehab Payer Rules We Manage

These are the rehab-specific rules our revenue cycle management team checks on every claim. For PT-only details, see our physical therapy billing services.

Discipline modifiers: GP, GO and GN

What it means: Medicare requires a modifier on every outpatient therapy service showing the plan of care it was delivered under: GP for physical therapy, GO for occupational therapy and GN for speech-language pathology. Assistant services also need CQ (PTA) or CO (OTA).

How we handle it: We match each service to the treating discipline and plan of care before the claim goes out.

KX threshold by discipline

What it means: Medicare sets one annual threshold amount for PT and SLP combined and a separate amount for OT. Above it, claims need modifier KX to confirm medical necessity is documented.

How we handle it: We track each patient’s spending by discipline, including services from other providers this year where visible, and add KX only when supported.

Multiple procedure payment reduction

What it means: When more than one therapy unit is billed on the same day, Medicare reduces the practice expense portion of the second and later units, across all disciplines.

How we handle it: We expect the reduction when posting payments, so true underpayments are not confused with MPPR and are appealed.

Plans of care and certification

What it means: Each discipline needs its own plan of care, certified by a physician or other qualified provider and recertified at least every 90 days, with progress reports at least every 10 treatment days.

How we handle it: We track certification and progress report dates by discipline so claims are not sent without them.

Institutional vs professional claims

What it means: Private practices bill on the CMS-1500, while rehab agencies, CORFs and hospital outpatient departments bill on the UB-04 with revenue codes 042X (PT), 043X (OT) and 044X (SLP).

How we handle it: We set up the right claim form, revenue codes and HCPCS lines for your facility type.

Home health and SNF therapy

What it means: Therapy in home health is paid under PDGM using OASIS data, and in skilled nursing facilities under PDPM, rather than per unit.

How we handle it: We support home health and SNF billing teams with documentation and claim checks for these payment models.

1. Eligibility, Visit Limits and Authorization

Before treatment we confirm therapy coverage, visit limits by discipline, deductibles, copays and prior authorization, and we track authorized visits so care is not interrupted.

2. Rehabilitation Coding by Discipline

Certified coders assign PT, OT and SLP CPT codes, timed units, discipline and assistant modifiers, and ICD-10-CM codes that support medical necessity. See our medical coding services and ICD-10 codes for physical therapy.

3. Clean Claim Submission

Claims are scrubbed for units, modifiers, NCCI edits and payer rules before submission, then tracked until paid, with rejections corrected the same day.

4. Denial Management and Appeals

Rehab claims are often denied for missing modifiers, wrong units, expired authorizations or documentation gaps. Every denial is worked through root cause analysis, corrected claims and appeals supported by the plan of care and treatment notes. See our denial management services.

5. Payment Posting, A/R Cleanup and Patient Billing

We post and reconcile payments, work old A/R, and send patients clear statements that explain therapy coverage, visit caps and deductibles.

Denials

Common Rehab Denials and How We Fix Them

Example: an OT service billed with the PT modifier

The denial. Therapeutic activities (97530) delivered by an occupational therapist are billed with modifier GP. The claim is denied or applied to the wrong threshold because the modifier does not match the OT plan of care.

How we fix it:

  1. Check the provider and plan. We confirm which discipline delivered the service.
  2. Correct the modifier. We resubmit with GO (and CO if an OTA was involved).
  3. Prevent the next one. Modifiers are mapped to each clinician’s discipline in your billing system.

Example: visits denied after an authorization expired

The denial. A commercial plan denies three speech therapy visits (92507) with CARC 197 (precertification or authorization absent) because the authorized visits ran out before the plan of care ended.

How we fix it:

  1. Request a retro review. Where the payer allows it, we ask for retroactive authorization with the progress notes.
  2. Appeal. If needed, we appeal with documentation of medical necessity.
  3. Prevent the next one. Authorized visits are tracked so renewals are requested before they run out.

FAQs

Rehab Medical Billing FAQs

What are rehab medical billing services?

They cover eligibility checks, authorization, coding, claim submission, denial management and payment posting for physical therapy, occupational therapy, speech-language pathology and multidisciplinary rehab care.

What are the GP, GO and GN modifiers?

They show which therapy plan of care a service was delivered under: GP for physical therapy, GO for occupational therapy and GN for speech-language pathology. Medicare requires one on every outpatient therapy service.

Is the KX threshold the same for PT, OT and speech therapy?

No. Medicare uses one combined threshold for PT and speech-language pathology and a separate threshold for occupational therapy.

What is the therapy multiple procedure payment reduction?

When more than one therapy unit is billed on the same day, Medicare reduces the practice expense part of payment for the second and later units.

Do you bill rehab agencies and hospital outpatient therapy?

Yes. We bill institutional claims on the UB-04 with therapy revenue codes as well as professional claims on the CMS-1500.
