Physical Therapy Billing

Physical Therapy and Physiotherapy Billing Services by Prospect Healthcare Solutions

Physical therapy billing specialist reviewing claims

Physical therapy billing services cover the coding, claims, denials and payments for outpatient rehab care: evaluations, timed treatment units, therapy modifiers and plan-of-care rules. Prospect Healthcare Solutions handles this full revenue cycle for physical therapy and physiotherapy clinics, private practices and multi-location rehab groups, so your therapists can spend their time on patients instead of paperwork.

PT claims look simple but carry strict rules. Units depend on documented minutes, Medicare needs GP, CQ and KX modifiers in the right places, and plans of care must be certified on time. We code every visit from the treatment note, check each payer’s rules and follow every claim until it is paid.

Why It's Different

Why Physical Therapy Billing Needs Special Expertise

Most physical therapy services are billed in timed units, so one missing minute or modifier can cut what you are paid or trigger a denial. Medicare, Medicaid and commercial plans each count units, limit visits and review medical necessity differently.

Common challenges include:

  • Converting treatment minutes to units correctly
  • GP, CQ, KX and 59/X modifiers applied in the right places
  • Plan-of-care certifications and progress reports on time
  • Visit limits and prior authorization on commercial plans
  • NCCI edits between therapy codes billed on the same day
  • Timely filing limits that differ by payer

Payer Rules

Physical Therapy Payer Rules We Manage

These are the therapy-specific rules our revenue cycle management team checks on every claim:

Timed codes and the 8-minute rule

What it means: Medicare counts units for timed codes such as 97110, 97112, 97140 and 97530 from the total timed minutes of the visit (8 to 22 minutes is 1 unit, 23 to 37 minutes is 2 units). Some commercial payers count each code separately.

How we handle it: We total the timed minutes in the note and bill the units each payer’s method allows.

GP and KX modifiers

What it means: Medicare requires modifier GP on every physical therapy service. Once a patient’s therapy costs pass the annual threshold Medicare sets each year, claims need modifier KX to confirm continued therapy is medically necessary and documented.

How we handle it: We track each patient’s therapy spending and add KX only when the note supports continued care.

Services by physical therapist assistants

What it means: When a physical therapist assistant provides all or part of a service, Medicare requires modifier CQ and pays 85% of the fee schedule amount.

How we handle it: We check who provided each service and apply CQ where the documentation shows assistant involvement.

Plan of care certification

What it means: Medicare requires a physician or other qualified provider to certify the plan of care, usually within 30 days of the first visit, and recertify it at least every 90 days. Progress reports are needed at least every 10 treatment days.

How we handle it: We track certification, recertification and progress report dates so claims are not sent without them.

Evaluations and re-evaluations

What it means: Evaluations are billed by complexity (97161–97163). A re-evaluation (97164) is paid only when there are new clinical findings or a significant change, not on a routine schedule.

How we handle it: We match the evaluation code to the documented complexity and bill re-evaluations only when the note supports one.

Procedures billed together

What it means: NCCI edits pair some therapy codes, such as manual therapy (97140) with therapeutic activities (97530). They are paid together only when performed in separate time periods, with modifier 59 or XU.

How we handle it: We check the timing in the note and add the modifier only when separate time is documented.

End-to-End

Our Complete Physical Therapy Billing Services

We manage the full therapy revenue cycle, from patient intake to final payment.

Physical therapy coding and claims review

1. Insurance Verification and Authorizations

Before the first visit we confirm coverage, therapy benefits, remaining visits, copays and any prior authorization, so treatment is not interrupted and claims are not rejected for eligibility.

2. Therapy Coding and Documentation Review

Certified coders assign CPT and ICD-10-CM codes, units and modifiers from each treatment note, and confirm the note supports every service billed. See our medical coding services and our guide to ICD-10 codes for physical therapy.

3. Clean Claim Submission

Claims are scrubbed for unit, modifier and NCCI errors before they go to Medicare, Medicaid and commercial plans, then tracked until paid, with rejections corrected the same day.

4. Denial Management and Appeals

Every therapy denial is worked through a structured process:

  • Root cause analysis (units, modifiers, authorization or documentation)
  • Corrected claims
  • Appeals supported by treatment notes and the plan of care

See our denial management services.

5. Payment Posting, AR Follow-Up and Billing Audits

We post and reconcile payments, follow up on unpaid claims, and audit coding, units and modifiers to find gaps before payers do. Learn about our medical billing audits.

Denials

Common Physical Therapy Denials and How We Fix Them

Example: manual therapy denied with therapeutic activities

The denial. Manual therapy (97140) and therapeutic activities (97530) are billed on the same day, and 97140 is denied with CARC 236 (this combination is not compatible under the payer’s coding edits).

How we fix it:

  1. Review the note. We check whether each service was performed in its own documented time period.
  2. Add the modifier. If the times were separate, we add modifier 59 or XU to 97140.
  3. Correct and resubmit. We send the corrected claim, or appeal with the treatment notes if the payer requires them.
  4. Prevent the next one. We check paired codes and documented times on every claim before it goes out.

Example: therapy denied after the annual threshold

The denial. A Medicare patient’s visit is denied with CARC 119 (benefit maximum for this time period has been reached) because the claim was sent without modifier KX after the patient passed the annual therapy threshold.

How we fix it:

  1. Check the documentation. We confirm continued therapy was medically necessary and documented.
  2. Resubmit with KX. If it was, we add KX and send the corrected claim.
  3. Check the ABN. If it was not, the visit is billed to the patient only when an ABN was signed before treatment.
  4. Prevent the next one. A threshold check runs on every Medicare therapy claim.

FAQs

Physical Therapy Billing FAQs

What are physical therapy billing services?

They cover insurance verification, coding, claim submission, denial management and payment posting for physical therapy and physiotherapy care, including evaluations, timed treatment units and therapy modifiers.

What is the 8-minute rule?

It is Medicare’s method for timed codes: total timed minutes for the visit are added up and converted to units, with at least 8 minutes needed for one unit.

When is the KX modifier used?

Once a patient reaches Medicare’s yearly therapy threshold, KX confirms that continued therapy is medically necessary and documented in the record.

What is the CQ modifier?

CQ marks Medicare services furnished in whole or in part by a physical therapist assistant. Medicare pays these at 85% of the fee schedule amount.

Is physiotherapy billing the same as physical therapy billing?

Yes. In the U.S., physiotherapy is billed as physical therapy, using the same CPT codes, GP modifier and payer rules.
