Behavioral Health Billing Services

Behavioral health programs bill differently from a typical therapy practice. Substance use treatment, intensive outpatient (IOP), partial hospitalization (PHP) and ABA therapy each have their own codes, authorization rules and documentation standards, and many of their claims go on a UB-04 instead of a CMS-1500.

Prospect Healthcare Solutions handles billing for these programs from start to finish: eligibility and authorizations, coding, claim submission, denial follow-up and payment posting. We also work within the extra privacy rules that apply to substance use records under 42 CFR Part 2.

Behavioral Health Programs We Bill For

  • Substance use disorder (SUD) treatment: outpatient counseling, medication-assisted treatment (MAT) and opioid treatment programs.
  • Intensive outpatient programs (IOP): mental health and SUD programs. Medicare covers IOP in hospital outpatient departments, community mental health centers, FQHCs, RHCs and opioid treatment programs when the care plan calls for at least 9 hours of services a week.
  • Partial hospitalization programs (PHP): structured day programs billed by hospitals and community mental health centers.
  • ABA therapy: assessments, technician-delivered treatment, protocol modification and caregiver training for autism services.
  • Community mental health centers and multi-program clinics that run several of these services under one roof.

We work inside your existing EHR and practice management system, track every claim and manage claim denials so revenue doesn’t stall.

Billing Rules by Program Type

SUD services. Medicaid and many state-funded programs use HCPCS H-codes, such as H0001 (alcohol and/or drug assessment), H0004 (behavioral health counseling, per 15 minutes), H0005 (alcohol and/or drug group counseling) and H0015 (alcohol and/or drug intensive outpatient). Commercial plans and Medicare often expect CPT or facility coding instead, so we match codes to each payer’s contract. Medical necessity is usually reviewed against ASAM level-of-care criteria.

IOP and PHP. Facility claims go on the UB-04 with revenue codes such as 0905/0906 (intensive outpatient, psychiatric / chemical dependency) and 0912/0913 (partial hospitalization). We track per-diem versus per-service billing, weekly hour requirements and concurrent reviews so authorizations don’t lapse mid-stay.

ABA therapy. ABA uses CPT 97151–97158, billed in 15-minute units, plus Category III codes 0362T and 0373T for destructive-behavior protocols. We check unit caps, technician and supervising-analyst credentials, and authorized hours before every claim. Revisions to the adaptive behavior codes have been proposed for 2027, and we will update your setup if they are finalized.

Every program also gets full revenue cycle management: verification, coding, claims, denials and payment posting.

HIPAA and 42 CFR Part 2 Compliant Billing

Substance use treatment records carry stricter privacy rules than other health records. Under the updated 42 CFR Part 2 rule, with a compliance date of February 16, 2026, patients can sign a single consent for treatment, payment and health care operations, and Part 2 breaches now follow HIPAA-style notification and penalties.

We bill within those limits, sharing only the information each claim needs and working in encrypted, access-controlled systems. The same HIPAA safeguards apply to every behavioral health and ABA account we handle.

Questions

Behavioral Health Billing FAQs

What does behavioral health billing cover?

Billing for substance use treatment, intensive outpatient programs (IOP), partial hospitalization programs (PHP), ABA therapy and community mental health centers. For outpatient therapy and psychiatry practices, see our mental health billing services.

How is behavioral health billing different from mental health billing?

Behavioral health programs often bill as facilities on the UB-04 with revenue codes, use HCPCS H-codes for Medicaid and state-funded services, bill per diem or per program day, and face level-of-care reviews. Substance use records also fall under 42 CFR Part 2.

Does Medicare cover intensive outpatient programs?

Yes. Medicare Part B covers IOP services in hospital outpatient departments, community mental health centers, FQHCs, RHCs and opioid treatment programs when the care plan calls for at least 9 hours of services a week. Patients generally pay 20% coinsurance after the Part B deductible.

Which CPT codes are used for ABA therapy?

ABA services use CPT 97151–97158, billed in 15-minute units, plus Category III codes 0362T and 0373T for destructive-behavior protocols. Revisions to these codes have been proposed for 2027.

What is 42 CFR Part 2 and does it affect billing?

42 CFR Part 2 protects substance use disorder treatment records. The updated rule, with a compliance date of February 16, 2026, allows a single patient consent for treatment, payment and health care operations, so billing must follow the consent on file and share only what each claim needs.
