Why Ambulatory Billing Needs Special Expertise
ASC claims are paid differently from physician claims. A single case can involve a facility fee, implants, multiple procedures and separate bills from the surgeon and anesthesia provider. Medicare, Medicaid, Medicare Advantage and commercial plans each apply their own covered procedure lists, authorization rules and fee schedules. One missed step can cost the whole case, so payer rules have to be checked before the patient arrives.
Ambulatory Surgery Payer Rules We Manage
These are the ASC-specific rules our revenue cycle management team checks on every case. Rules vary by payer and change often, so we confirm each one against current payer policy.
Covered procedure lists
What it means: Medicare pays an ASC facility fee only for procedures on its ASC covered procedures list. Commercial plans keep their own lists and site-of-service policies.
How we handle it: We check each scheduled CPT code against the payer list before the case, and flag anything that may not be payable in an ASC.
Prior authorization
What it means: Many commercial and Medicare Advantage plans require authorization for the procedure, the facility and sometimes the implant.
How we handle it: We confirm that the authorization matches the CPT codes, date, facility and surgeon, and update it when the plan for the case changes.
Multiple procedure reductions
What it means: When several procedures are done in one session, payers commonly pay the highest-ranked procedure in full and reduce the others.
How we handle it: We sequence lines correctly and check the payment against the contract or fee schedule, so underpayments are found and appealed.
Implants and packaged services
What it means: Medicare packages many implants and supplies into the ASC payment, while some commercial contracts pay for implants separately, often with an invoice.
How we handle it: We bill implants according to each contract, attach invoices when required, and track whether they were paid correctly.
Modifiers and discontinued procedures
What it means: Laterality, bilateral and distinct-procedure modifiers affect payment. Modifiers 73 and 74 report facility procedures discontinued before or after anesthesia.
How we handle it: We review the operative report before assigning modifiers, and use them only when the documentation supports them.
Place of service and separate billing
What it means: ASC facility claims use place of service 24, and the surgeon and anesthesia provider bill their professional services separately. Some commercial payers also ask for modifier SG.
How we handle it: We match claim details across the case and follow each payer format, which helps avoid mismatches and duplicate denials.
Our Complete Ambulatory Billing Services
We manage the full ASC revenue cycle, from scheduling and benefit checks to final payment.
1. Eligibility and Authorization Verification
Before surgery we verify coverage, deductibles, out-of-pocket costs and authorization, so patients know their costs and cases are not denied for avoidable reasons. Learn more about insurance eligibility verification.
2. Ambulatory Coding and Documentation Review
Certified coders assign CPT, HCPCS and ICD-10-CM codes from the operative report, with modifiers and NCCI edits checked on every line. See our medical coding services.
3. Clean Claim Submission and Follow-Up
Every claim is checked before submission, then tracked with the payer until it is paid, so no case is forgotten or left unpaid.
4. Denial Management and Appeals
Every denial is worked through root cause analysis, corrected claims and appeals with the operative report and authorization attached. See our denial management solutions.
5. Payment Posting and Patient Billing
We post and reconcile payments against your contracts, send clear patient statements, and follow up on balances. Read about our accounts receivable services.
Common Ambulatory Billing Denials and How We Fix Them
Example: a case denied for missing authorization
The denial. A knee arthroscopy (29881) is denied with CARC 197 (precertification or authorization absent).
How we fix it: We check whether authorization was obtained and whether it matches the billed code and date. If it exists, we correct the claim. If not, we request a retro-authorization where the payer allows it, or appeal with the medical records.
Example: a second procedure denied as bundled
The denial. A secondary procedure on a multi-procedure claim is denied with CARC 97 (service included in another procedure).
How we fix it: We review NCCI edits and the operative report. If the procedure was truly separate, we add the correct modifier and resubmit. If it was bundled, we correct the claim and use the finding to improve future coding.
Specialized Ambulatory Billing Services
Multi-Specialty ASCs
We bill cases across ophthalmology, orthopedics, GI, pain management, ENT and podiatry, with each specialty’s rules applied to every claim.
Implant-Heavy Procedures
We track implants from the operative report to the invoice and the payment, so high-cost devices are billed under each contract. See our orthopedic billing services.
GI and Pain Procedures
We bill endoscopy and spinal injection cases, including screening versus diagnostic rules. See our gastroenterology and pain management billing pages.
Ambulatory Surgery CPT Codes, HCPCS Codes and Modifiers
These are common ASC codes and modifiers we work with. Examples only: coverage and payment vary by payer, so we confirm each one before billing.

Ambulatory Billing FAQs
What are ambulatory billing services?
They cover eligibility checks, authorization, coding, claim submission, denial management, payment posting and patient billing for same-day surgery and outpatient procedures, usually for ambulatory surgery centers.
How is ASC billing different from physician billing?
An ASC bills a facility fee for the room, staff and supplies. The surgeon and anesthesia provider bill their professional services separately.
What place of service code does an ASC use?
Ambulatory surgical centers use place of service 24 on the claim.
Are implants paid separately in an ASC?
It depends on the payer. Medicare packages many implants into the ASC payment, while some commercial contracts pay for them separately.
What do modifiers 73 and 74 mean?
They report facility procedures discontinued before anesthesia (73) or after anesthesia began or the procedure started (74).
