Plastic Surgery Payer Rules We Manage
In plastic surgery, how a claim is billed matters as much as which codes are on it. These are the payer rules we check before each claim goes out:
Cosmetic and reconstructive work in one surgery
What it means: When a patient has a covered reconstructive procedure and an elective cosmetic one in the same session, the payer covers only the reconstructive part. Operating room, anesthesia and surgeon fees have to be split.
How we handle it: We bill the reconstructive codes to insurance, set up a signed self-pay agreement for the cosmetic part before surgery, and make sure the operative note separates the two.
Medicare and cosmetic exclusions
What it means: Medicare does not cover cosmetic surgery at all. An ABN is not required for a service Medicare never covers, but the claim must show it is excluded if it is sent.
How we handle it: We append modifier GY when a cosmetic service has to be reported to Medicare, so the denial goes to the patient correctly and the claim isn’t held for review.
Payer medical policy criteria
What it means: Payers publish criteria for procedures such as reduction mammaplasty, panniculectomy and blepharoplasty, often including photos, symptoms that have lasted for a set time, and failed conservative treatment.
How we handle it: We match the chart to the payer’s current policy before the authorization request, and tell the practice which items are missing.
Global surgical periods
What it means: Most major plastic surgery procedures have a 90-day global period. Follow-up visits are included, and another procedure during that time is denied unless the right modifier explains it.
How we handle it: We use modifier 58 for staged procedures (such as a tissue expander exchange), 78 for an unplanned return to the operating room, and 79 for an unrelated procedure.
Breast reconstruction and symmetry
What it means: The Women’s Health and Cancer Rights Act also requires coverage of surgery on the other breast to produce a symmetrical appearance, and of prostheses.
How we handle it: We bill symmetry procedures on the opposite breast with RT or LT and the diagnosis codes that show they are part of the mastectomy reconstruction.
Multiple procedures and bilateral surgery
What it means: When several procedures are done in one session, payers pay the highest-valued one in full and reduce the rest. Bilateral procedures use modifier 50 and are paid at a set percentage.
How we handle it: We list procedures from highest to lowest value with modifier 51 where the payer uses it, and bill bilateral work the way each payer requires.
Excision and repair coding
What it means: Intermediate and complex repairs can be billed in addition to a lesion excision. Repairs of the same complexity in the same anatomic group are added together and billed as one code.
How we handle it: We add repair lengths by complexity and site, and check that the note documents the size and layers of each repair.
A Common Plastic Surgery Denial and How We Fix It
Example: an upper eyelid blepharoplasty denied as cosmetic
The denial. A blepharoplasty (15823) comes back with CARC 50 (not medically necessary) because the payer considers it cosmetic.
How we fix it:
- Check the payer’s policy. We compare the chart to the policy’s criteria, which usually include visual field testing with the eyelids taped and untaped and photographs showing the lid covers the pupil.
- Collect what is missing. If testing or photos were done but not sent, we gather them with the physician’s notes on the patient’s symptoms.
- Appeal. Our denial management team files the appeal with the test results, photos and a letter showing how the procedure meets each criterion.
- Prevent the next one. We use a pre-authorization checklist for each procedure with a medical policy, so the request is complete the first time.
Example: a return to the operating room denied in the global period
The denial. A patient returns to the operating room for a hematoma after breast surgery, and the claim is denied with CARC 97 (included in the payment for another service) because it falls in the 90-day global period.
How we fix it: a complication that needs a return to the operating room is billed with modifier 78, and an unrelated procedure with modifier 79. We correct the claim with the right modifier and the diagnosis for the complication, and resubmit. Complications treated in the office are part of the global package and are not billed.
Plastic Surgery Codes for Reconstructive, Breast and Eyelid Procedures
Plastic surgery billing starts with one question: is the procedure cosmetic or reconstructive?
| Service | Codes and rules | What we check |
|---|---|---|
| Cosmetic vs reconstructive | Cosmetic procedures are self-pay; reconstructive procedures need documented functional impairment | Coverage is confirmed and cosmetic fees are collected before surgery |
| Blepharoplasty | 15822 and 15823 upper eyelid | Visual field testing and photos show the field loss payers require |
| Breast surgery | 19318 reduction mammaplasty; reconstruction after mastectomy, which group plans must cover under the Women's Health and Cancer Rights Act | Authorization and supporting criteria such as symptoms and tissue weight are documented |
| Panniculectomy | 15830 | Documented skin conditions or functional problems support medical necessity |
| Grafts and tissue transfer | 15100–15261 skin grafts; 14000–14350 adjacent tissue transfer by site and size | Defect size and site are documented |
| Prior authorization | Required for most reconstructive procedures | Approvals match the planned codes |
Plastic Surgery Billing FAQs
Does insurance cover plastic surgery?
Only reconstructive procedures that correct a functional problem, such as eyelid surgery that improves vision. Cosmetic procedures are self-pay.
What do payers need for blepharoplasty?
Visual field testing and photographs showing the eyelid blocks the patient's vision.
Is breast reconstruction after mastectomy covered?
Yes. Under the Women's Health and Cancer Rights Act, group health plans that cover mastectomy must cover reconstruction.
