Dermatology Payer Rules We Manage
Dermatology billing involves more than procedure codes: pathology, specialty drugs, phototherapy and coverage limits each have their own payer rules. These are the rules our medical billing team manages beyond the codes:
Dermatopathology billing
What it means: When a practice reads its own slides, it can bill the pathology code (such as 88305) globally, or split it into professional (26) and technical (TC) parts when another lab prepares the slides. In-house labs also need a CLIA certificate.
How we handle it: We confirm who prepared and who read each specimen, bill the right component, and check the lab’s CLIA number on the claim.
Biologics and specialty drugs
What it means: Biologics for psoriasis, atopic dermatitis and other conditions usually need prior authorization, often with step therapy (trying other treatments first) and renewal every few months.
How we handle it: We track approval and renewal dates, gather the treatment history payers ask for, and bill in-office drugs with the right HCPCS code, units and NDC.
Phototherapy
What it means: Phototherapy (96910, 96912) and excimer laser treatment (96920–96922, by treatment area) are billed per session. Many payers require authorization and limit the number of sessions.
How we handle it: We check session limits before treatment starts, track sessions used, and request extensions before the limit is reached.
Mohs surgery coverage
What it means: Mohs codes can be billed only when the same physician acts as both surgeon and pathologist. Medicare contractors also list the tumor types and sites where Mohs is appropriate.
How we handle it: We check that the Mohs report documents the indication, stages and tissue blocks, and that the surgeon read the slides.
Full-body skin exams
What it means: Medicare does not cover routine skin cancer screening for patients without symptoms or a history of skin cancer. A visit for a specific problem or a history of skin cancer is billed as a regular E/M.
How we handle it: We code the visit from the reason documented, and report a routine screening to Medicare with modifier GY so the patient is billed correctly.
Hyperhidrosis treatment
What it means: Botulinum toxin injections for excessive sweating (64650 with the drug code) need prior authorization and proof that topical treatments failed. Cosmetic uses of the same drug are self-pay.
How we handle it: We request authorization with the treatment history, bill the drug units given plus any wasted amount with the JW modifier, and keep cosmetic injections off insurance claims.
A Common Dermatology Denial and How We Fix It
Example: an office visit denied on the same day as a biopsy
The denial. A patient comes in with a changing mole and a rash. The dermatologist does a punch biopsy (11104) of the mole and starts treatment for the rash. The office visit is denied with CARC 97 (included in the payment for another service).
How we fix it:
- Review the note. The decision to biopsy is part of the procedure, so we check whether the visit also documents separate work, such as the evaluation and treatment of the rash.
- Correct and resubmit. If it does, we send a corrected claim with modifier 25 on the visit and the rash diagnosis linked to it.
- Leave it off when it isn’t separate. If the visit covered only the lesion that was biopsied, the visit isn’t billed, and we share a short documentation tip with the provider.
- Prevent the next one. We review every visit billed with a same-day procedure before the claim goes out.
Example: removal of seborrheic keratoses denied as cosmetic
The denial. Destruction of benign lesions (17110) comes back with CARC 50 (not medically necessary), because the payer treats removal of seborrheic keratoses as cosmetic.
How we fix it: if the note documents symptoms such as bleeding, itching, inflammation or irritation from clothing, our denial management team appeals with the note and the diagnosis codes for those symptoms. If it doesn’t, the service is the patient’s responsibility when they agreed in writing beforehand. Going forward, we flag benign lesion removals so symptoms are documented or the self-pay agreement is signed before treatment.

Dermatology Codes and Rules We Handle
Dermatology claims depend on precise procedure selection: biopsy technique, lesion type, site and size all change the code. These are the dermatology codes and rules we work with most:
| Service | Codes and rules | What we check |
|---|---|---|
| Biopsies | 11102 tangential, 11104 punch, 11106 incisional, each with an add-on code (11103, 11105, 11107) for additional lesions | The biopsy method in the note matches the code, and add-ons are counted correctly |
| Destruction of lesions | 17000, 17003 and 17004 for premalignant lesions such as actinic keratoses; 17110 and 17111 for benign lesions such as warts | Lesion counts match the code ranges |
| Excisions | Benign 11400–11471 and malignant 11600–11646, coded by site and excised diameter including margins; intermediate and complex repairs billed separately | Pathology results are in before malignant codes are used, and repair length is documented |
| Mohs surgery | 17311–17315 by site and number of stages and tissue blocks | Stage and block counts match the Mohs report |
| Same-day E/M and procedures | Modifier 25 for a significant, separately identifiable E/M; modifier 59 or XS for separate lesions | The E/M note supports more than the pre-procedure assessment |
| Cosmetic vs medically necessary | Removal of benign lesions such as seborrheic keratoses is often treated as cosmetic unless symptoms are documented | Symptoms such as bleeding, irritation or inflammation are in the note, or the patient is told it is self-pay |
