Why It's Different

Why Anesthesia Billing Needs Special Expertise

Anesthesia is paid by a formula rather than a flat fee: base units plus time units, multiplied by a conversion factor. Who performed the service also changes the claim, because anesthesiologist, CRNA and medically directed cases use different modifiers. Small errors in start time, stop time or provider details can reduce payment or deny a case.

Payer Rules

Anesthesia Payer Rules We Manage

These are the anesthesia-specific rules our revenue cycle management team checks on every claim. Rules vary by payer and change often, so we confirm each one against current payer policy.

Base and time units

What it means: Payment is usually built from base units plus time units, commonly counted in 15-minute increments, then multiplied by a conversion factor.

How we handle it: We confirm anesthesia start and stop times on the record and check how each payer counts time.

Provider modifiers

What it means: AA, AD, QK, QX, QY and QZ show who provided the service and whether it was personally performed, directed or supervised.

How we handle it: We match the modifier to the anesthesia record and staffing for each case.

Medical direction rules

What it means: Medicare medical direction allows up to four concurrent cases and requires specific steps to be met and documented.

How we handle it: We check concurrency and required documentation before using direction modifiers.

Monitored anesthesia care

What it means: MAC claims use modifiers such as QS, G8 and G9, and many payers review medical necessity closely.

How we handle it: We report the right MAC modifier and confirm the record supports the service.

Physical status and qualifying circumstances

What it means: Modifiers P1 to P6 report physical status, and codes such as 99100 to 99140 report qualifying circumstances. Not every payer pays extra for them.

How we handle it: We report them when documented and apply each payer policy.

Separately billable services

What it means: Some lines, such as arterial lines or a post-operative pain block requested by the surgeon, may be billable when documentation supports them.

How we handle it: We review the record for missed services and apply NCCI edits and modifier rules.

1. Eligibility and Authorization Verification

Before the case we verify coverage and the plan rules for the facility and procedure, so claims are not denied for avoidable reasons. Learn more about insurance eligibility verification.

2. Anesthesia Coding and Documentation Review

Certified coders assign anesthesia codes, modifiers and time from the anesthesia record, and check them against the surgical procedure. See our medical coding services.

3. Clean Claim Submission and Follow-Up

Every claim is checked before submission, then tracked with the payer until it is paid, so nothing is forgotten or left unpaid.

5. Payment Posting and Patient Billing

Denials

Common Anesthesia Denials and How We Fix Them

Example: a case denied for a missing provider modifier

The denial. A CRNA case is denied with CARC 4 (modifier inconsistent or missing) because no QX or QZ was reported.

How we fix it: We confirm the staffing model on the record, add the right modifier and submit a corrected claim.

Example: a directed case denied as a duplicate

The denial. The anesthesiologist and CRNA claims for one case are matched and one is denied with CARC 18 (duplicate).

How we fix it: We confirm QK and QX are reported on the correct lines, correct the claims and resubmit.
