Audiology Billing

Audiology Billing Services by Prospect Healthcare Solutions

Medical billing support team

Audiology billing services cover the coding, claims, denials and payments for hearing and balance care: diagnostic hearing tests, vestibular testing, cochlear implant programming and hearing aid services. Prospect Healthcare Solutions handles this full revenue cycle for independent audiologists, ENT clinics, hearing aid centers and multi-location practices, so your team can focus on helping patients hear better.

Audiology mixes covered diagnostic tests with services many payers exclude, such as hearing aids and routine hearing exams. We code every service from the record, apply each payer’s coverage rules and follow every claim until it is paid or correctly billed to the patient.

Why It's Different

Why Audiology Billing Needs Special Expertise

Audiology claims depend on who ordered the test, which ear was tested, and whether the payer covers the service at all. Medicare, Medicaid, Medicare Advantage and commercial plans each treat hearing care differently.

Common challenges include:

  • Order requirements and the Medicare AB modifier
  • Bilateral codes billed for one ear
  • Hearing aids and routine exams excluded by Medicare
  • Hearing aid benefits that vary by plan
  • Bundled test codes billed separately in error
  • Device and supply HCPCS codes

Payer Rules

Audiology Payer Rules We Manage

These are the audiology-specific rules our revenue cycle management team checks on every claim:

Orders and the AB modifier

What it means: Medicare usually requires a physician or NPP order for diagnostic audiology tests. Audiologists may perform certain tests without an order for non-acute hearing conditions once every 12 months, billed with modifier AB.

How we handle it: We check for an order or AB eligibility, and track the 12-month limit by patient.

Bilateral codes and modifier 52

What it means: Most audiometry codes describe testing both ears. When only one ear is tested, the code is reported with modifier 52 for a reduced service.

How we handle it: We read which ears were tested and apply 52 where needed, never RT/LT or 50 on bilateral codes.

Hearing aids and Medicare exclusions

What it means: Original Medicare does not cover hearing aids or exams for fitting them. Many Medicare Advantage, Medicaid and commercial plans do, with their own limits.

How we handle it: We verify hearing benefits up front, bill excluded services with modifier GY where appropriate, and bill the plan that covers the device.

Diagnostic vs treatment services

What it means: Medicare pays audiologists for diagnostic tests, not for treatment such as aural rehabilitation, which may be covered only when provided by other qualified clinicians.

How we handle it: We check each service against the payer’s rules for audiologists before billing.

Bundled test codes

What it means: Some codes include others. For example, 92557 includes pure tone and speech audiometry, and 92550 includes tympanometry with reflex testing.

How we handle it: We bill the comprehensive code when it applies and do not unbundle its parts.

Vestibular and implant services

What it means: Vestibular tests (such as VNG) and cochlear implant programming have specific codes, age ranges and frequency rules.

How we handle it: We match codes to the test components and patient age, and document medical necessity.

1. Eligibility and Hearing Benefits Verification

Before the visit we verify coverage, hearing aid benefits, limits and authorization requirements, so patients know their costs and claims are not denied for eligibility.

2. Audiology Coding and Documentation Support

Certified coders assign CPT, HCPCS and ICD-10-CM codes for tests, devices and visits, with modifiers checked on every line. We also help your team keep documentation that supports each code. See our medical coding services.

3. Clean Claim Submission and Follow-Up

Every claim is checked before submission, then tracked with the payer until it is paid, so no claim is forgotten or left unpaid.

4. Denial Management and Appeals

Every audiology denial is worked through root cause analysis, corrected claims and appeals with supporting documentation. See our denial management services.

5. Payment Posting and Patient Billing

We post and reconcile payments, send clear patient statements for hearing aids and other patient-paid services, and follow up on balances.

Denials

Common Audiology Denials and How We Fix Them

Example: a hearing test denied for missing order

The denial. Comprehensive audiometry (92557) for a Medicare patient is denied with CARC 16 (claim lacks information) because no ordering provider was reported and modifier AB was not used.

How we fix it:

  1. Check the record. We look for a physician or NPP order.
  2. Correct the claim. We add the ordering provider, or modifier AB if the test qualifies and none was billed in the past 12 months.
  3. Prevent the next one. Orders and AB dates are checked at scheduling.

Example: a hearing aid fitting denied by Medicare

The denial. A hearing aid fitting is billed to Original Medicare and denied with CARC 96 (non-covered charge).

How we fix it:

  1. Check other coverage. We look for a Medicare Advantage, Medicaid or supplemental plan with a hearing benefit and bill it.
  2. Bill the patient correctly. If no plan covers it, the service is billed with GY and the patient is billed under your pricing.
  3. Prevent the next one. Hearing benefits are verified before device appointments.

FAQs

Audiology Billing FAQs

What are audiology billing services?

They cover eligibility checks, coding, claim submission, denial management, payment posting and patient billing for hearing tests, balance testing, cochlear implant services and hearing aids.

What is the AB modifier in audiology?

Modifier AB lets audiologists bill Medicare for certain diagnostic tests without a physician order, for non-acute hearing conditions, once every 12 months.

Does Medicare cover hearing aids?

Original Medicare does not cover hearing aids or exams for fitting them. Many Medicare Advantage, Medicaid and commercial plans offer hearing aid benefits.

How do you bill a hearing test for one ear?

Most audiometry codes are bilateral, so testing one ear is reported with modifier 52 for a reduced service.

Do you bill vestibular and cochlear implant services?

Yes. We bill vestibular tests such as VNG and cochlear implant programming, matched to the components performed and the patient’s age.
